Ms. Gersh notes that the Trump administration has canceled or reduced the Centers for Medicare and Medicaid mandatory pilot projects that tie reimbursement to quality and efficiency of care. The challenge with voluntary value-based health care programs is that participants tend to be elite health systems that are skilled at delivering high quality low cost care, so the pilot projects may not generate much additional savings, Ms. Gersh observes.
